Hereditary Breast, Ovarian and Pancreatic Cancer VCEP
The c.8486C>T variant in ATM is a missense variant predicted to cause substitution of proline by leucine at amino acid 2829 (p.Pro2829Leu). This variant has been detected in individual(s) with Ataxia-Telangiectasia (personal communication; PMID: 9711876; PMID: 37438524). This variant has been reported to segregate with Ataxia-Telangiectasia in two affected individuals from one family (personal communication). The highest population minor allele frequency in gnomAD v4.1.0 is 0.00001667 in the Admixed American population (PM2_Supporting, BS1, and BA1 are not met). The computational predictor REVEL gives a score of 0.952, which is above the threshold of 0.733, evidence that correlates with impact to ATM function. In summary, this variant meets the criteria to be classified as likely pathogenic for autosomal dominant ATM-related cancer predisposition and autosomal recessive Ataxia-Telangiectasia based on the ACMG/AMP criteria applied as specified by the HBOP VCEP. (PM3_Strong, PP1, PP3)
Met criteria codes
PM3_Strong
This variant has been detected in 3 individuals with Ataxia-Telangiectasia. Of those individuals, two were confirmed in trans by family testing (personal communication). One individual was homozygous for the variant (PMID: 9711876; PMID: 37438524)(PM3_Strong).
PP1
This variant has been reported to segregate with Ataxia-Telangiectasia in two affected individuals from one family (personal communication)
PP3
The computational predictor REVEL gives a score of 0.952, which is above the threshold of 0.733, evidence that correlates with impact to ATM function (PP3). SpliceAI does not predict any aberrant splicing associated with this variant.
Not Met criteria codes
PM2
The highest population minor allele frequency in gnomAD v4 is 0.000016 (1/60002 alleles) in Admixed American population which is higher than the ClinGen HBOP threshold 0.001% for PM2_Supporting
